Living Healthy Alternatives Inc. – Appointment Booking Policy
Effective Date: 07/23/25
By booking an appointment with Living Healthy Alternatives Inc. (“LHA”), the client agrees to the following terms:
Scheduling
Appointments may be scheduled by phone, in person, or through our online system. All bookings require a valid payment method to reserve your time slot. Confirmation will be sent by text or email.
Cancellations & Rescheduling
Clients must cancel or reschedule at least 24 hours before the appointment time.
Appointments canceled with proper notice are eligible for a full refund minus the deposit, or may be rescheduled at no cost.
Cancellations made less than 24 or 12 hours before the appointment will be charged 25% or 50% of the session fee, respectively.
No-shows (missed appointments without notice) are non-refundable and may not be rescheduled without a new payment.
Late Arrivals
Clients arriving more than 10 minutes late may receive a shortened session. Clients arriving 15 minutes late or more may be considered a no-show and charged accordingly.
Refund Policy
Full Refunds are only issued for appointments canceled more than 24 hours in advance. Cancellations made less than 24 hours before the appointment may be eligible for a 50% refund, only if it is before the scheduled time.
No-shows or cancellations without any notice are non-refundable. Partial refunds are not guaranteed and are reviewed case by case.
Clients who purchase multi-session packages must refer to the Prepaid Package Policy for specific refund terms.
Health Disclosure
Clients must inform staff of any medical conditions, injuries, or other relevant health information prior to services. LHA does not diagnose or treat medical conditions and services are not a substitute for medical care.
Liability Release
By booking an appointment, clients voluntarily assume all risks related to services received. Clients release and hold harmless Living Healthy Alternatives Inc., its owners, staff, and affiliates from all claims or liabilities, including but not limited to injury, emotional distress, or illness related to services.
No Guarantee of Results
LHA makes no promises or guarantees regarding outcomes. Wellness services affect individuals differently.
Governing Law
This agreement is governed by the laws of the State of Illinois. Disputes shall be resolved through binding arbitration in Lake County, IL.
By booking, the client affirms they have read and agree to these terms.
